Summary
This list of duties and responsibilities, which follows, represents the broad range of tasks which may be required to be undertaken either routinely or periodically. This list is not exhaustive and the role may include additional duties which are not listed here. Communication 1) Communicates effectively as a member of a multi-disciplinary team using a variety of media a) Refers when necessary to general practitioners, Advanced Nurse Practitioners, Practice Nurses, HCAs, Reception & Admin Team, Paramedics, Accident and Emergency, Secondary Care, District nurses, Community MDT, social workers, mental health team, and Social Care Teams. 2) With Primary care nurses giving support and advice. 3) With reception staff regarding patient information. 4) Recognise and effectively manages situations where there are barriers to effective communication. 5) With patients (a) facilitates a range of non -medicinal therapies including lifestyle change and social prescribing (b)Promotes personalised shared decision making to support adherence and concordance with treatment. Clinical Skills 1) Undertakes skills and roles expected of a Treatment Room Practice Nurse. Role including a. Undertake the collection of pathology specimens including intravenous blood samples, urine samples and swabs b. Injections within competency and training, and within practice protocol including but not limited to VitB12,,GnRHA given under appropriate PGDs. c. Wound care and Suture and surgical clip removal d. ECGs e. Hypertension Monitoring- including ambulatory monitoring f. NHS health checks g. Sexual health screening h. Therapeutic drug monitoring /Near patient testing i. Immunisations and vaccinations within the national immunisation program including childhood and adult immunisations e.g.-Whooping Cough for pregnancy, influenza, pneumococcal, shingles.Childhood Vaccinations and travel vaccinations after further training j. Provides advice for lifestyle change including smoking cessation, alcohol and weight management as brief interventions Following additional training treatment room nurses should also provide :- k. Cervical Cytology l. Compression bandaging and Doppler assessments m. Learning disability health checks n. Contraceptive advice /pill checks 0. CHD and anticoagulation monitoring p. Travel health q. Asthma / COPD chronic disease management r. Diabetes chronic disease management. 2, Delivers a high standard of care without discrimination to all ages, sexes and ethnic origins of the population through continuum of life; working collaboratively across agency boundaries to improve health outcomes and reduce health inequalities 3. Functions at a high level in relation to communication skills through patient consultation and nursing assessment of physical, psychological and social needs, promoting health and wellbeing. Supporting patients to self-care and self-manage. 4.Deliver safe, effective evidence based care and advice to ease patient problems including when to escalate care and ensuring appropriate safety netting .Ensure safe and appropriate follow up arrangements 5. Adapts to change within working situation including managing unscheduled emergency situations. 6. Performs safe manual handling techniques. 7. Recognises own limitations, seek assistance and refers appropriately. Organisational Skills 1) Aware of and complies with CH policies and procedures. 2) Able to adapt to change in the workplace being involved in new developments in the delivery of care. 3) Ensures faulty clinical equipment is reported and repaired and adequate for use. 4) Ensures store and medicine supplies are adequately ordered and maintained. 5) Participates and supports the implementation of clinical governance, clinical audit and clinical risk assessment, setting and monitoring standards of care. 6) Promotes improving the health of the community through reporting and preventing occurrences of infectious disease, proving advice and education. Professional 1) Ensures practice and implementation of care is evidence based by adhering to local and national guidance. 2) Complies with Nursing and Midwifery council code of conduct and works within scope of professional practice 3) Maintains confidentiality of information regarding patients. 4) Ensures accurate, concise documentation. 1) Reports any adverse incidents or near miss events through agreed CHoC system. 2) Maintains professional development in risk awareness, health and safety, fire safety at work, Child and Adult safeguarding, conflict resolution, information governance and basic life support by attending annual mandatory training. 3) Identifies and/ or participates in strategies of risk assessment to avoid or prevent clinical incidents and near misses incorporated in the clinical governance framework including: a) Risk management, Child and Adult safeguarding policies, Drug policies and Infection prevention policies. Training and Educational 1) Is responsible for and enhances professional development. 2) Continually keeps up to date with current evidence based practice. 3) Is involved in lifelong learning. 4) Ensures mandatory training has been completed. 5) Participates in in-house training provided / clinical meetings / supervision 6) Supports practice development by acting as a mentor/clinical assessor as appropriate
