Summary


To work within the Community Stroke Service on a Bank contract providing specialist nursing assessment and treatment for patients following a diagnosis of stroke.

This role will assist in the assessment and treatment of patients with newly diagnosed stroke, within the West Norfolk locality, who have been discharged.

This role will sit within the West Norfolk community stroke service maximising patients optimal level of independence.

• To in-reach into The Queen Elizabeth Hospital Stroke services where necessary and work collaboratively with the wider stroke care systems in the Integrated Care System, Acute, and Community services to provide a service to West Norfolk patients.
• Provide expert advice to relevant personnel on the management of stroke, ensuring a seamless service across the stroke pathway.
• Providing highly specialist nursing assessments and advice
• To provide medication reviews, basic observations and liaise with professionals as required
• Communicating complex and sensitive information to patients and carers as to their assessment, prognosis and treatment plan. To work frequently with patients and families in complex situations that are emotionally distressing and may cause hostility. This may include completing suicide risk assessments and providing and receiving contentious information.
