Summary

Main Duties and Responsibilities: Frailty prevention - Support frail people to remain independent for longer by providing evidence-based Comprehensive Geriatrics Assessments (CGAs) in the community responding cohesively alongside the wider MDT. Integrated neighbourhood working - Regular attendance at the Integrated Neighbourhood team MDT. When required supporting the smooth running of these meetings as Chair. Clinical Leadership & Frailty Care Work cohesively with the wider MDT, including Palliative and EOL care teams in the community, to coordinate holistic, person-centred care and ensure[EF1] smooth transitions across pathways. Assist in monitoring and auditing clinical practice to ensure adherence to patient safety, and quality standards. Team Leadership & Line Management Support the Nurse Manager with the line management of Nursing and HSW staff, including day-to-day oversight, supervision, and delegation of tasks. Ability to travel across INT sites to support clinical and supervisory activity Foster a positive, compassionate, and supportive culture that encourages teamwork, psychological safety, and professional growth. Contribute to the induction, supervision, and ongoing professional development of team members. Other responsibilities: Service Redesign & Improvement Support the identification of service development needs and assist in implementing redesign initiatives to enhance patient outcomes and operational efficiency. Work with the Nurse Manager to mobilise new programmes of work and supporting with clinical oversight as appropriate. Work collaboratively with the Nurse Manager and partnersincluding Palliative and EOL teamsto refine care pathways, ensuring they reflect best practice and meet patient needs. Promote innovation and the integration of new services and technologies. Quality, Safety & Governance Participate in clinical audits and quality improvement activities. Support local investigations of incidents and events, ensuring learning is identified and actions are implemented. Contribute to the development and review of risk assessments aligned with organisational policies. Promote patient involvement in care decisions and respond to patient and family concerns in a supportive and timely manner. Education & Staff Development Support staff learning needs and assist in delivering training or signposting to development opportunities. Help ensure all new staff receive appropriate induction and mandatory training. Management, Leadership and Administrative Responsibilities Workforce & Resource Management Under the direction of the Nurse Clinical Manager:[EF2] Monitor and escalate staffing levels, workload, and resource allocation. Contribute to recruitment processes, including shortlisting, interviewing, and onboarding. Support the development of staff retention initiatives, including mentorship and career development programmes. Performance & Reporting Assist with monitoring and analysing key performance indicators (KPIs) relating to clinical outcomes, staffing, and service delivery. Contribute to reports for senior management and regulatory bodies. Support performance review processes, helping to identify strengths and development needs within the team. Escalate concerns regarding staff performance and support HR-aligned processes where required, under the direction of the Nurse clinical manager.[EF3] Communication & Team Coordination Support effective communication between Nursing and HSW teams, the Nurse Manager, and the broader leadership group. Maintain clear documentation of incidents, complaints, and clinical events in line with reporting protocols. Policy and Service Development responsibilities: Contribute to the development of patient information to support patient care. Actively contribute to the clinical governance agenda, ensuring compliance with statutory and mandatory training. Have knowledge of policy and practice changes to improve service delivery. Lead by example in fostering an inquisitive approach and driving a culture change within clinical practice, in line with the patient safety incident response framework, to enable the delivery of excellent person-centred care and support for families/carers. Participate fully in meetings, clinical supervision, and other key meetings. Develop, review, and update policies and procedures to ensure they are in line with current best practices, legal requirements, and organisational goals. Ensure that all policies are communicated effectively to the team and are consistently followed in practice. Research and Audit Responsibilities: Utilise research findings to inform and enhance clinical practice. Support with data collection for service evaluation and audit. Identify and address personal development needs, staying current with new developments in the field. Regularly audit clinical practices to ensure compliance with internal procedures and national regulations. Education and Training responsibilities: Educate team members and partners on how to support individuals to plan for their future care through promoting, supporting and encouraging advanced care planning and use of the Respect form. Develop skills in delivering training sessions for healthcare workers, enhancing skills and knowledge to improve care delivery across Nimbuscare's services. Supervise and mentor students during clinical placements, providing constructive feedback and supporting professional growth.
