The HCSW is part of the multidisciplinary team and plays a vital role in our multidisciplinary team, providing holistic, patient- centred care which is planned and directed by health care professionals. The post holder is expected to cover the duties of other HCSW colleagues within the range of the post holder’s knowledge and skills.

The HCSW will complete performance necessary patient care tasks delegated by a registered nurse or midwife, under their supervision such as undertaking personal care duties and record patient information.

Undertake personal and professional development activities to uphold care standards, such as completing the 15 standards of the Care Certificate and the associated workbook within a timeframe of normally 206 months from commencement of employment.

Follow all London North West University Healthcare NHS Trust (LNWH) policies and provide a safe and person-centred environment for the treatment of patients.

The post holder is responsible for delivering high standards of personal care, providing services in accordance with the Trusts values and behaviours to support the nursing team in the delivery of patient care which is safe and effective.
