Summary
Advanced Clinical Practice The ACP will assess, diagnose, plan and implement treatment for patients within their care, acting within their own scope of competence to do so. Using critical clinical consultation skills they will provide care for a wide range of patients.Predominantly working as an independent and autonomous practitioner as part of the multi-disciplinary team providing safe and effective person-centred care Evidence and Research The ACP will contribute to and undertake audits of clinical practice to ensure care provision is in line with guidelines for best practice. The ACP will participate in and contribute to team exercises in the improvement of patient care and will assist in dissemination of new guidelines and recommendations for practice, evaluating the effectiveness of any changes to practice if appropriate. Training and personal development Education and Learning: The ACP will positively contribute to their own learning and development by maintaining up to date skills and knowledge, undertaking additional training as required, this may be identified by the individual or the practice. They will also participate in appraisal/performance reviews annually and undertake mandatory/statutory training as required. The ACP undertakes personal responsibility for ensuring that their own professional requirement are met fully including revalidation and annual prescribing reviews where necessary. Leadership personal and people development Leadership: Acting as a positive role model for others, the ACP will offer support to colleagues in their development, acting as a mentor to junior colleagues/student nurses/medical students as required promoting a positive learning environment within the practice. They may also contribute to the development and review of local guidelines and protocols. The ACP will assess their own performance and take accountability for own actions, either directly or under supervision and contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance Administration and professional responsibilities Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures Participate in the administrative and professional responsibilities of the practice team Accurate and legible notes of all consultations and treatments are recorded in the patients notes Ensure clinical systems kept up to date, recording and/or amending accurate details Ensure appropriate items of service claims are made accurately, reporting any problems to the Deputy Practice Managers Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice The post-holder must regard all information relating to patients and their carers, practice staff and other healthcare workers (as well as information relating to the practice as a business organisation) as strictly confidential and manage this information in line with professional standards for conduct and legal requirements including (but not exclusive to) GDPR, information governance and data protection legislation.
