Summary

Some of the main operational duties and responsibilities include: Assist the clinical leads to manage the nurse-led leg ulcer service, meeting the needs of patients within the contracted localities. This includes triaging referrals, monitoring/managing waiting lists and assessing patients in line with contracted requirements. Support the clinical leads in the provision of the necessary training for nurses within Hertfordshire in evidence-based leg ulcer management through the development, implementation and evaluation of theoretical and practical training programmes as appropriate. Promote a leg ulcer service philosophy which embraces a holistic approach to patients, relatives and carers. Take responsibility for coordinating the delivery of localised specialised leg ulcer care as required by the service. Monitor and support all grades of community staff working within leg ulcer clinics. Support the practice development of other nurses within GP surgeries, community and nursing homes. Act as a resource and ensure continuation of appropriate leg ulcer care for patients in other environments e.g. home, hospital. Whilst it is not a legal requirement, Hertfordshire Community NHS Trust encourages and supports all staff to be vaccinated as this remains the best line of defence against COVID-19.
