Summary
Job Description Job Title: Practice Nurse Accountable to: Practice Manager (administrative) Nurse Manager (clinical) Hours: As agreed (primarily treatment room and long-term conditions clinics) Job Summary To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice, including care for elderly and housebound patients. Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population. The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, well woman and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure. The organisation will ensure the post holder has access to appropriate clinical supervision and a named individual to provide advice and support on a day-to-day basis. Collaborative Working The role requires effective multidisciplinary team working, clear communication, appropriate delegation, and shared responsibility for service delivery. Managing Information Accurate clinical record keeping using appropriate clinical systems and SNOMED coding is required to support audit and service monitoring. Security and Professional Conduct Staff must adhere to security procedures, professional standards, dress appropriately, and maintain safe working practices. Primary Key Responsibilities Support infection prevention and control requirements, including audits, training, and evidence of compliance Support the publication of the IPC annual statement Develop and embed care management programmes for frail, elderly, and housebound patients Assess patient needs and deliver evidence-based care Implement and evaluate treatment plans for patients with long-term conditions Provide routine nursing care in line with NICE and national standards Deliver wound care, vaccinations, chronic disease clinics, and health promotion Maintain accurate clinical records and registers Support safeguarding and clinical emergencies Liaise with external agencies to support vulnerable patients Secondary Responsibilities Participate in audit and quality improvement activity Support the development and review of nursing protocols Participate in shared learning and service development Contribute to significant event reviews and clinical governance processes Equality Statement The job description may be amended following consultation to support service development and individual role progression.
