Highly Specialist Cardiac Pharmacist (Supporting Primary Care)

(37.5 hours a week including back-up on call and weekend commitments

An exciting opportunity has arisen for a motivated and enthusiastic pharmacist to join the cardiac team at Barts Heart Centre within Barts Health NHS Trust.

The post holder will work alongside the cardiovascular prevention primary care pharmacy teamand deliver quality care to patients through efficient medicines optimisation. The post provides several opportunities for professional and personal development, contributing to quality improvement projects and supporting the delivery of world-class and innovative cardiovascular services

There is a need for close coordination with the cardiovascular prevention primary care pharmacy team and wider primary care MDT such as general practitioners, pharmacy professionals, nurses, practice managers, community pharmacy, etc to plan workload and priorities. This involves optimising cardiovascular medicine, for long term conditions related to cardiac disease across primary care services within North-East London (NEL) and beyond.

The post holder will work with primary care teams to continually develop the service, while maintaining high standards and achieving best practice to meet the needs of patients and the Trust.

The post holder communicates effectively with patients to help optimise their pharmaceutical management and through liaising with other health care professionals to design, implement plans to optimise health outcomes.
