Description
We are seeking a dedicated and compassionate Through-Care Navigator (Level 6) to join our team and make a significant impact in the lives of individuals navigating complex care pathways. In this essential role, you will serve as a bridge between clients and necessary medical, psychological, and social services, ensuring that they receive comprehensive support throughout their journey. Your ability to communicate effectively and empathize with clients will be key as you assist them in understanding available resources, finding optimal solutions to their unique challenges, and fostering a collaborative environment where they feel empowered. With an emphasis on continuity of care, you will work closely with a diverse range of health professionals, community organizations, and families to ensure seamless transitions between service providers, ultimately helping clients achieve their wellness goals. This position requires a deep understanding of social services systems and an unwavering commitment to client advocacy. By building trusting relationships and providing reliable support, you will play a crucial role in enhancing the overall well-being of those we serve. Responsibilities
Serve as the primary point of contact for clients throughout their care journey.
Assess client needs and develop personalized care plans in collaboration with multidisciplinary teams.
Navigate clients through various health and social service systems to facilitate access to resources.
Monitor and evaluate client progress, adjusting care plans as necessary to meet evolving needs.
Provide education and support to clients and families regarding available services and healthcare options.
Advocate on behalf of clients to ensure their voice is heard in care decisions and processes.
Develop and maintain strong relationships with community partners to enhance resource availability and quality of care.
Requirements
Bachelor's degree in social work, public health, nursing, or a related field; Master’s degree preferred.
Minimum of three years of experience in care coordination, case management, or a related field.
Strong understanding of healthcare systems, social services, and community resources.
Exceptional communication and interpersonal skills, with the ability to build rapport with clients from diverse backgrounds.
Proficient in data management and documentation practices, with experience using electronic health record systems.
Demonstrated ability to work independently, manage multiple cases, and prioritize effectively in a fast-paced environment.
Strong problem-solving skills and a compassionate approach to client care.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:30
Pay Per Hour: £18.43
