Summary
Job Purpose Virtual wards support patients, who would otherwise be in hospital, to receive the acute care, remote monitoring, and treatment they need in their own home or usual place of residence. Virtual wards provide acute clinical care at home for a short duration (up to 14 days) as an alternative to care in hospital Planning, co-ordinating and delivering care as appropriate per the daily MDT, initiate and manage medical and nursing care interventions, lead care and co-ordinate partner agencies and stakeholders. To provide high quality holistic evidence-based care to patients / service users that is responsive, effective and efficient in line with the virtual ward model To facilitate and support timely assessment, treatment and effective discharge from the virtual ward and prevent inappropriate admissions by supporting patients / clients to remain as independent as possible and maintain or improve their level of function and quality of life The Virtual Ward core hours are 08.00 20.00 over 7 days. Role Context To provide appropriate and effective care management through the virtual ward pathway unnecessary hospital admission from the Community managing the active caseload and providing necessary oversight and support To holistically assess the needs of patients and carers, plan, implement and evaluate nursing care interventions. Embrace multi professional and partnership working to facilitate co-ordination, promotion of optimum health and social outcomes independence and self-care. Undertake virtual monitoring and escalate concerns to the daily MDT. Will be responsible for holistic assessments, implementation of care, overall co-ordination of self-care management or care management. Take responsibility for leading the shift on a regular basis Key Responsibilities Key Accountabilities To focus on people who would otherwise face unnecessarily prolonged hospital stays or inappropriate admission to acute inpatient care, long term residential care or continuing NHS in-patient care. To deliver high quality care to patients / service user on the community virtual ward To assess, plan and implement treatment programmes and take responsibility to ensure this is evaluated and amended as appropriate. To take lead with patient / service user management under direction of Advanced Clinical Practitioners/ acute consultants and lead clinicians . To provide education and advice service to patients / service users promoting independence and self-management techniques. To work at all times within the Codes of Professional Conduct. To encourage and promote flexibility and innovation within the team to meet the needs of the caseload population. To facilitate Public Health Initiatives by educating patient / service user on their health and well-being to promote independence whilst on the virtual ward. To lead and co-ordinate timely discharge from the virtual ward and prevent inappropriate hospital admissions. To act as Role Model to all team members. Maintain accurate and contemporaneous documentation to ensure timely, effective communication and data collection. Ensure that patient / service user documentation clearly demonstrates the organisational Guidelines for Records and Record Keeping. Be personally accountable for maintaining skills, knowledge and competence in the use of SystmOne Be aware of the Data Protection Act to ensure appropriate action is taken to always ensure confidentiality and the protection of patient / service user identifiable information. The post holder must ensure that caring with compassion is at the core of their practice. The post holder will act as a role model and will utilise their leadership skills to ensure that all staff treat patients in a compassionate manner demonstrating respect for their individual needs, privacy, and dignity Champion the roles and values of the virtual ward at all levels of the organisation and to a variety of health and social care professionals. Self-evaluate practice to ensure evidence-based care. Contribute to the wider development of practice by participating in research, audit, local and national presentations, networks, and publications. Educate and empower patients, carers, and care home staff to identify deterioration in condition and provide them with the necessary knowledge and skills to make informed choices to safely manage their condition. Develop robust care plans with patients and carers to assist in the management of their condition and promote independence whilst on the virtual ward. Participate in the future training of health students and staff and other professional groups. COMMUNICATION Effectively communicate with the daily MDT, virtual ward colleagues, wider Integrated Teams, patients, and carers to ensure that the patients needs are identified, addressed, and appropriate information and advice is provided on discharge from the virtual ward. Facilitate effective communication between Primary, Secondary, Community, Social Care and Voluntary settings to promote best possible outcomes. Maintain high standards of documentation in line with NMC and trust policies and procedures. Ensure the implementation of the Virtual Ward concept for management of the most complex patients within the community. PERSONAL, PROFESSIONAL DEVELOPMENT Actively engage in clinical supervision and mentorship Maintain a personal development plan and professional portfolio. Maintains professional development, encompassing mandatory requirements. Ensure revalidation is maintained. HEALTH, SAFETY AND SECURITY To be responsible for the promotion of best practice that ensures the health, safety and security of self, patients, and colleagues in accordance with the policies, procedures, and guidelines of the trust. Practice in accordance with the procedures and guidelines for the protection of vulnerable adults and child protection. Ensure that confidentiality is protected at all times in accordance with data protection.
