Summary
Undertake comprehensive initial and review Children & Young Peoples Continuing Care (CYPCC) Assessments in line with the National Framework (2016) Support CYPCC eligible individuals and or their representatives working collaboratively with families, carers and multi-disciplinary teams to design and implement personalised care packages. Monitor and review care packages to ensure they remain responsive to changing needs. Contribute to service development, quality improvement and safeguarding initiatives across Gloucestershire. Support the development and review of CYPCC Personal Health Budgets (PHB) in line with the CYPCC National Framework (2016). Ensure accurate and open communication and co-ordination with a range of organisations and individuals, research and draft correspondence and papers and ensuring the management of specific tasks, lead reporting and analysis across a range of specialties, functions and projects including contributing to policies, procedures and pathways and CCC local and National Frameworks. Support in the strategic development and oversight of local transition planning processes and planning meetings. Undertaking CYPCC assessments and care planning to ensure smooth transition to adult services. Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally.
