Summary

Access team: The post holder will: Be based in Access CAMHS in Springfield site, assisting with triage, assessment and sign posting ensuring Children and Young People reach the most suitable pathway within the locally agreed time frames Offer telephone, e-consultation and face to face assessment to young people and families who experience mental health/emotional/neurodevelopmental difficulties. Take part in triage rota, processing new referrals and making initial brief triage calls to young people and their carers/parents to discuss concerns, assess risk, and develop initial care plans as needed Join a multi-disciplinary team at Wandsworth Access Functions of the post will include high quality assessments; formulation, and consultation with multi-agency professionals as needed Support integration of assessment where Children and Young People present with Mental Health / Emotional Well Being and there are also social contextual issues Have an agreed job plan with specified activity and throughput levels; Co-ordinate care for those children, young people and families on their caseload; Manage risk within team, Directorate and Trust structures; Work to relevant professional and ethical guidelines, and within the framework of Directorate and Trust policies and procedures. Use screening and outcome measures in clinically meaningful ways to demonstrate treatment effectiveness and seek feedback about user experience. Tier 3 team: Join a multi-disciplinary team at Tier 3 Wandsworth Carry out high quality assessments. Provide evidence-based treatment. Have an agreed job plan with specified activity and throughput levels. Co-ordinate care for those children, young people, and families on their caseload.
