The team
Home First provides short-term reablement care and support to people living in their own homes. Our service helps individuals regain independence following a hospital stay, illness, or crisis at home, and also works to prevent unnecessary hospital admissions.
You’ll collaborate with colleagues across Adult Community Services (ACS) Cluster Teams, Independent Providers, Health partners, Borough and District Councils, Multi-Agency Safeguarding, and a wide range of voluntary sector organisations.
This is a hybrid role, with midweek office days dedicated to team collaboration, sharing best practice, and supporting each other. Every other weekend, you’ll work from home, covering the Home First county alongside colleagues from other area teams.
Home First is a fast-paced environment where the work we do makes a real difference to people’s lives. Our team is enthusiastic, committed, and guided by SCC’s We Aspire values, creating a dynamic and rewarding workplace.
Office days will be agreed at interview to meet both business and individual needs.
