Summary
Developing personalised programmes of care for service users alongside other team members, which include psychology, medical, pharmacy, nursing and support staff. OT staff are responsible for focusing on developing structured goal orientated interventions to address issues all areas of functioning and to increase independence. Carrying out specific OT assessments to support care planning and any discharge needs. This may include specialised assessments to support funding packages or specialised intervention. To work closely with other partner organisations to develop community based resources and to address barriers for service users to access and develop support networks The OT staff within our STR service are engaged in QI projects looking at improving service users and families experience of services and are actively engaged in service improvements at all levels. As the OT in the team, you will have responsibility to assess and develop OT interventions , using a range of assessment and guidelines, including MOHOST and formulations. You will be responsible for the assessment, planning, implementation and evaluation of rehabilitation interventions including upskilling and improving ADL, social & financial and budgeting skills, physical and mental health interventions for service users who will have a diagnosis of complex psychosis. Support and guidance with housing & accommodation, recreational and vocational interventions. You will have the direct support of a Band 7 Lead OT, OT Consultant, other OT S2R staff across the service and activity coordinator.
