Summary

Clinical Care Provide direct care and support to individuals with mental health needs and/or learning disabilities, ensuring their physical, emotional, and social well-being in a variety of care settings. Assist in the development and implementation of personalised care plans in collaboration with the service user, family, and wider care team. Monitor and record physical and mental health conditions, escalating concerns to the registered nurse or clinical lead as necessary. Support therapeutic interventions, such as group activities, behavioural therapies, or life skills training. Identify risks using contemporary assessment tools to improve safety and quality of care. 2. Communication and collaboration Build positive, respectful relationships with service users, families, and carers. Work effectively within a multidisciplinary team, maintaining open and professional communication. Advocate for the needs and preferences of service users, ensuring their voices are heard and respected. Recognise changes in a person's condition that require intervention and understanding when to refer to the supervising Registered Nurse and senior colleagues for reassessment. Report and raise concerns about Safeguarding, accessing advice and support at the point of need. Be conversant with Trust policies, procedures and systems for safeguarding including the assessment of Mental Capacity, the process for requests for Deprivation of Liberty Safeguards (DOLS) and demonstration of the principles of Prevent (counter terrorism awareness). 3. Professional Practice Maintain accurate and up-to-date documentation in line with organisational policies and professional standards. Adhere to regulatory frameworks, including those of the Nursing and Midwifery Council (NMC) and organisational policies. Participate in training, supervision, and reflective practice to ensure personal and professional development. Support in Education and Training Contribute to the education and training of service users, families, and staff, sharing knowledge and promoting awareness of mental health and learning disabilities. Act as a role model for best practices in care delivery and professional behaviour. To provide supervision to newly qualified staff and students, new starters and Student Nursing Associates encouraging an effective learning environment. To provide support and supervision to Healthcare Assistants to promote their learning and development within their role.
