If so, we want you to join our Short Term Assessment and Reablement Team in Amber Valley area of Derbyshire. As a Social Care Practitioner, you will play a vital role in supporting adults to regain independence following illness, injury, or a change in circumstances. Working closely with individuals, families, and multi-disciplinary professionals, you will assess needs, develop person-centred plans, and provide short-term interventions that empower people to live safely and confidently in their own homes. What You’ll Do
  • Carry out holistic assessments and reviews in line with Care Act principles.
  • Work collaboratively with health professionals, therapists, and community services.
  • Promote independence and wellbeing through tailored reablement plans.
  • Ensure safeguarding and risk management are at the heart of your practice.
  • Use strength-based approaches to achieve positive outcomes.
About You
  • Experience in adult social care or a related field.
  • Strong communication and problem-solving skills.
  • Ability to work flexibly and manage a varied caseload.
  • Commitment to promoting dignity, choice, and independence.
  • Knowledge of relevant legislation and best practice.
