If so, we want you to join our Short Term Assessment and Reablement Team in Amber Valley area of Derbyshire.
As a Social Care Practitioner, you will play a vital role in supporting adults to regain independence following illness, injury, or a change in circumstances. Working closely with individuals, families, and multi-disciplinary professionals, you will assess needs, develop person-centred plans, and provide short-term interventions that empower people to live safely and confidently in their own homes.
What You’ll Do
- Carry out holistic assessments and reviews in line with Care Act principles.
- Work collaboratively with health professionals, therapists, and community services.
- Promote independence and wellbeing through tailored reablement plans.
- Ensure safeguarding and risk management are at the heart of your practice.
- Use strength-based approaches to achieve positive outcomes.
- Experience in adult social care or a related field.
- Strong communication and problem-solving skills.
- Ability to work flexibly and manage a varied caseload.
- Commitment to promoting dignity, choice, and independence.
- Knowledge of relevant legislation and best practice.
