Crynodeb
Carry own caseload and undertake holistic assessments including risk of the child/adolescent/family needs, either individually or in identified complex cases jointly with another member of the CYPS Mindworks.Provide short term treatment to CYP.To liaise with Social Care, Education and other appropriate agencies/professionals /carers to ensure best outcomes for CYP.Act as a lead professional for CYP following initial assessment. Developing an appropriate plan of care.Offer support for Students on placementCo-work with other members of the team MDT.Take responsibility for maintaining accurate and timely records of all work undertaken in accordance with Trust policies and Professional body requirements.Provide statistical information on all activities undertaken, in line with Trust procedures. Have basic IT skills and Data entry skills to enter clinical information on the database.Contribute to the teams overall standard of services provided.Promote effective liaison with all statutory and voluntary agencies concerned with CYP and mental health.Through the appraisal and supervision systems, we identify own training needs and undertake continuous professional development.Ensure high standards of care are provided, and that of dignity, individuality, self-esteem and wellbeing of individuals and their families are given. Monitored and evaluated on a continuous basis.
