Band 8a dependent on experience HOURS Part-time 28 hours RESPONSIBLE TO: The Partners The Wycliffe Medical Practice are seeking to employ a part-time independently prescribing Advanced Nurse Practitioner, with the ideal candidate being someone who is proactive, enthusiastic and happy to work as part of an outstanding, long established team. Practice Profile We are based in a large purpose built site in the town of Lutterworth, and pride ourselves on providing a friendly, patient centred service. 10, 900 patient list size SystmOne The clinical team includes 5 partners, 5 Salaried GPs, 1 ANP, 4 Nurses, 1 Nurse Associate, 3 HCAs, 2 Physiotherapists and 1 Admiral Nurse. We have an Senior Clinical Pharmacist backed up by outstanding PCN Pharmacist Support. Experienced reception and administrational team. JOB SUMMARY: Work as an autonomous practitioner as part of a multi-professional team in the delivery of high quality care to registered patient list of approximately 10,900 patients Triage, assess, diagnose and treat patients requesting same day appointments, or refer to GP nursing team for further intervention management Able to manage own caseload in response to unpredictable patterns of work. Act as a clinical expert in direct contact care. Facilitates expert inter-professional care for the ill patient and their dependants by direction, supervision and example; and maintenance of all records. We are looking for excellent clinical skills, coupled with outstanding communication and a hands on, patient focussed approach. You will need to be able to demonstrate safe and competent clinical decision making as well as outstanding assessment and diagnostic skills. You must also be forward thinking and able work collaboratively within our friendly team. Key responsibilities will be: To assess, diagnose, plan, implement and evaluate treatment/interventions. Diagnose and manage both acute and chronic conditions. Clinically examine and assess patient needs and plan clinical care accordingly. Care for patients presenting with an undifferentiated diagnosis and be able to refer appropriately. Support patients to adopt health promotion strategies that promote healthy lifestyles Demonstrate appropriate communication ensuring patients are fully informed and consent to treatment The ideal candidate will also have: Applicants should be autonomous practitioners with experience in primary care and minor illness. Able to independently Prescribe at least 2 years experience of working as an Advanced Nurse Practitioner within General Practice Minor Illness and Injury qualifications We welcome applications from nurses, ANPs & ECPs A Knowledge of SystmOne, Clarity Teamnet and would be an advantage however being a Fast learner with the ability to learn a variety of IT based systems is essential. 1. DUTIES & RESPONSIBILITIES General 1.1 Continue to develop, expand and role model own personal clinical expertise as autonomous practitioner. 1.2 Develop own areas of specific clinical expertise in order to link acute, primary, community and emergency care sectors in expanding nursing practice. 1.3 To ensure that the practices Clinical Governance requirements are delivered including the development of sound clinical practices. 1.4 Coordinates and manages the delivery of inter-professional care. 1.5 Ensures the prompt instigation of appropriate treatment following consultation with the relevant care groups. 1.6 Monitors the care of patients with other team members to ensure that care is implemented and remains appropriate to the needs and wishes of the patient. 1.7 Provides and receives complex, sensitive or contentious information, where persuasive, motivational, negotiating, training, empathic or reassurance skills are required. This may be because agreement or co-ordination is required or because there are barriers to understanding. 1.8 Demonstrates effective communication skills in highly distressing or emotional circumstances, this includes: terminal illness, child abuse and family breakdown. Clinical 1.9 Analyse, monitor and evaluate standards of care and facilitate remedial action when necessary. 1.10 Deliver specialised care in keeping with the needs and wishes of patients and their families. 1.11 Ensures patient care is delivered in accordance with established policies and protocols and to the agreed standard. 1.12 Contribute to the development and maintenance of clinical policies 1.13 To prescribe effectively using the local formulary as appropriate. 1.14 Suturing of patients wounds, following assessment. 1.15 Contribute to developments in the Practice in response to Practice objectives and develops practice accordingly. 1.16 Analyse complex situations and to make an objective interpretation and rationale, to achieve the best solution. 1.17 Request and interpret a range of blood tests/microbiology/radiology tests. 1.18 To carry out venepuncture if required. 1.19 Knowledge of the cardiac arrest procedures, anaphylactic shock, violent incidents fire regulations, and, ensuring that appropriate action is taken in response to such an emergency. 1.20 To be able to maintain accurate electronic and written patient records for users of services provided. 1.21 To be aware of responsibilities under the Data Protection Act. 1.22 To participate in regular multi-professional team meetings. 1.23 To ensure effective communication with nursing and medical colleagues. 1.24 To treat everyone with whom you come into contact, with dignity and respect. 1.25 Promote Equality & Diversity and a non-discriminatory culture. 1.26 Identify and take action when other peoples behaviour undermines Equality and Diversity. Management 1.27 Supporting the practice manager to report and answer complaints and untoward occurrences. 1.28 To take clinical responsibility for themselves 1.29 To assist in the development of clinical standards, protocols and procedures and in the efficient running of the service. 1.30 To be aware of the political and social issues which affect the delivery of healthcare. 1.31 Promote effective management of change by acting upon audit and other reviews to implement agreed new approaches to raise the quality of care1.32 To participate in clinical commissioning and peer review with other members of the clinical team. Educational 1.33 To work closely with other members of the team to ensure staff are developed to a competent and high standards. 1.34 Participate in the overall evaluation of clinical outcomes of the registered patient population . 1.35 Assist in the development of new models of working across clinical boundaries, in particular with community nursing and Allied Health Professionals as well as the Local Authority . 1.36 To participate in research, clinical supervision and audit. 1.37 Provide professional leadership and act as a role model, coaching and teaching staff, to raise understanding, standards of professionalism and contribute to the development of nursing. 1.38 Identify learning needs and implement training in relation to safe use and maintenance of equipment . 1.39 Provide consistent and ongoing support for patients and their families in self management, empowering them to understand and manage their own health needs and when to seek the interventions of health care professionals 1.40 Maintains own personal and professional development in accordance with NMC and other national guidelines. Audit and Research 1.41 Assists with and facilitates data collection activities and participation in clinical audit. 1.42 Participate with clinical research programmes as appropriate 1.43 Apply and evaluate research and development findings in nursing practice to improve the standards of care delivered. 1.44 Assist in the standard setting initiatives, protocols, practice policies and quality assurance programmes and implementation within the practice 1.45 Committed to the maintenance and development of all information systems in the clinical area. 1.46 Reviews and revise the systems, practices and skills of the inter-professional team through the system of Clinical Governance to ensure they enable the delivery of the best possible care. 1.47 Report all complaints and near misses, untoward events and work with the practice team towards a culture of lessons learned and reducing risk of reoccurrence . 1.48 Encourage critical appraisal of research findings amongst team colleagues.
