Summary

The Locality Coordinator will support the Health Squad Programme with the overall development, implementation and delivery of the service. Due to the nature of the clients, often being part of hard to reach groups / communities there will be a requirement to work flexibly including some weekends and evenings. - Line manage a team of Advanced Wellbeing Practitioners - Support the development and promotion of the Health Squad offer on community asset based approaches and in line with the County Durham Together vision - Encourage and develop person centred health plans with clients to address identified risk factors - Support vulnerable clients to optimise their health and wellbeing - Engage with clients and offering them support and brief intervention advice and offering physical health checks, including, finger prick sampling, BP readings, body measurements, and undertaking mental wellbeing assessment tools - Support clients to understand and manage their health and wellbeing through behaviour and lifestyle changes assisting them to achieve their goals, manage their condition and undertake prehabilitation - Liaise with relevant services, organisations and support networks to develop both client pathways and support packages for clients - Carry out service evaluations in line with the project plan and learning from best practice - Address health inequalities through asset based, holistic conversation, identifying support from family and wider community - Ensure appropriate information and resources are available for staff and members of the public, in order for individuals to make changes to their behaviour that have a positive impact on their health and wellbeing - Represent the Health Squad at key stakeholder / community group meetings and work closely with the Advanced Practitioners to implement key actions and feedback.
