Summary
Working collaboratively with the Mental Health Practitioners and the general practice team to meet the needs of patients - supporting in the triage of assessments, delivering interventions and education - to manage patients with a variety of mental health conditions. You will engage with patients to provide support and time limited interventions to meet their health and social needs - with an emphasis on empowerment and recovery. Working closely with the Mental Health Practitioners to devise plans of care to support recovery and advocate improved choice - using a strengths and asset approach to harness and improve patients own network to improve resilience. Experience of working in the community in a mental health setting would be beneficial. Primary care experience is not essential; however, a good understanding of how Primary Care works would be beneficial. Most importantly, the right candidate will have a strong desire to achieve the best possible outcomes for patients and have the motivation and flexibility to help develop services to meet the needs of the population. The role will be embedded within the PCN however it will be outward facing and will link seamlessly with North Staffordshire Combined Healthcare Trust where you will be supported with supervision, and training opportunities etc.
