Summary
The post holder will work within their clinical competencies and the requirements of the DES to: Provide clinical support for long-term condition management, including BP and cholesterol management in line with local and national guidance, and CKD reviews with monitoring, optimisation of medicines and patient education. Undertake structured medication reviews and ongoing medication reviews for patients with long-term conditions and/or polypharmacy, with a focus on de-prescribing, reducing medicines related harm and improving outcomes. Manage day to day clinical queries and tasks via AccuRx and other practice systems, ensuring timely, safe and appropriate responses to patients and colleagues. Contribute to the safe and efficient flow of medicines related information by managing Docman administration, including medicine related actions from hospital and outpatient letters and discharge summaries. Implement and act on medicines safety alerts and MHRA drug safety updates, supporting the practices to maintain robust medicines safety processes. Undertake DOAC monitoring and other high risk drug monitoring, ensuring appropriate blood tests, followup and clinical review, and documenting actions clearly in the clinical record. Perform CQC related prescribing and safety searches, and support the practices with audits, recalls and followup actions to maintain safe and effective care. Support repeat prescribing processes, medicines reconciliation (including post discharge) and other prescribing/administrative tasks as required, ensuring consistency with local formulary and prescribing guidance. Contribute to service development and quality improvement work within the PCN, undertaking other appropriate duties as identified by the PCN and in response to the needs of the practices population.
