The Intensive community Support for Dementia East (ICSDE) is looking for a full-time Band 5 Registered Mental Health Nurse to join this busy and dynamic team based at Alderney Hospital in Poole and covering east Dorset.

ICSDE is a multi-professional team of Nursing staff, with Medical and Occupational Therapy input, and is a great mix of autonomous and teamwork where everyone's contribution is valued. The team is highly regarded, very supportive of each other and has its own in-house teaching programme, as well as Team Reflective Practice sessions and has been involved in research projects to enable improvements in practice.

This is a specialist community dementia team; predominantly psycho-social led which offers a broad service. The team supports the Older People’s Community Mental Health Teams in managing their most complex cases and are highly effective in preventing unnecessary admissions to the organic wards at Alderney Hospital. They do this through the provision of rapid, specialist and intensive short-term support at home.

The team works closely with carers and other agencies in older people’s health and social care services and supports the discharges of people with dementia from the organic wards into community placements. They work closely with Alderney Day Hospital and the ICSD team in the west of the county.



We are looking for a Staff Nurse who wants a fulfilling role where they can use their skills and knowledge to make a positive difference to people living with dementia in Dorset. Ideally, we are looking for RNMHs but if you are an experienced RGN or RNLD with evidence of community and dementia experience then your application would be welcomed.

We are looking for a dynamic and highly motivated nurse, with good assessment, risk management and care formulation skills who ideally has experience of working with older adults and dementia health in a multi-disciplinary setting. A high standard of communication skills and clinical decision-making is essential in this role. ICSDE is a ‘virtual ward’ and nursing staff must be able to process information in a variety of ways accurately and quickly.

The ability to work flexibly across a range of weekends, early mornings and evening shifts is also important as well as the work of the service depends on the fluidity and adaptability of the whole team.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
