To strengthen the Hospital Discharge Team and ensure timely, safe, and effective discharges, I propose employing two locum workers in the role of Social Work Officer (akin to Assistant Social Worker). These positions will provide critical support in a fast-paced environment, working collaboratively with social workers, health professionals, and discharge coordinators.
Key Purpose of the Role:
Support hospital discharge functions by facilitating assessments, care planning, and coordination of services.
Maintain a physical presence across key hospital sites, including:
Whittington Hospital
Royal Free Hospital - North Middlesex Hospital
Rehabilitation beds under Pathway 2
Out-of-borough hospitals as required
Assist in managing complex discharge pathways, ensuring compliance with statutory duties and local protocols.
Contribute to reducing delayed transfers of care and improving patient flow.
Responsibilities:
Work under the supervision of Team manager and Assistant Team Manager as well as qualified social workers to gather information, liaise with families, and coordinate community support.
Support discharge planning meetings and ensure accurate documentation.
Engage with multidisciplinary teams to expedite safe discharges.
Monitor and follow up on care arrangements post-discharge.
Skills and Attributes Required:
Ability to work in a high-pressure, fast-moving environment.
Strong communication and organisational skills.
Knowledge of hospital discharge processes and community care pathways.
Flexibility to travel across multiple hospital sites.
