Summary
The post holder will utilise specialist skills and knowledge as an Advanced Clinical Practitioner to autonomously manage patients undergoing SACT in all clinical environments. The post holder will possess the skills and knowledge to be able to assess individuals and families using person-centred approaches and a range of assessment methods including history taking, physical examination, ordering and interpreting diagnostic tests or conducting health needs assessment.Apply advanced clinical-reasoning skills to conduct comprehensive assessments of presenting complaints, instigate and interpret diagnostic tests, and develop both working and differential diagnoses. Take responsibility for formulating, communicating, implementing, and evaluate the effectiveness of management plans. Use a diverse range of knowledge to achieve competence in evidence based therapeutic interventions, which will include prescribing medication (where legally allowed) and actively monitoring their effectiveness. To promote value-based clinical decision making, critically evaluating and appropriately applying, their knowledge and skills in a person-centered way. You will be instrumental in the development and implementation of local and network wide management and treatment protocols and pathways, challenging predetermined protocols or workplace imperatives where necessary.
