Summary
- Manage the Digital Clinical Hub: Oversee the processing of clinical correspondence for all aligned Southport and Formby GP practices. -Utilise EMIS Functionality: Ensure efficient and accurate recording of clinical information using EMIS software. -Line Management: Provide leadership and people management to a team of Digital Clinical Hub Care Coordinator Team Leads. - Reporting: Deliver service reports to Southport and Formby PCN, including Key Performance Indicators (KPIs) and other monitoring requirements. - Team Development: Support the team in accessing relevant training, development, and supervision sessions. - Resource Management: Assist in the creation and management of EMIS resources for the digital hub. - Liaison and Communication: Maintain continuous communication and engagement with PCN practices and other stakeholders via the regular steering group and wider group meetings. This is an evolving role within the PCN, and additional responsibilities will be added over time as the service evolves. The individual must also be willing to undertake the appropriate training as set out by the Personalised Care Institute as part of the national framework for Care Co-Ordinators. The individual must be happy to work with both clinical and non-clinical colleagues. The individual must have experience of using clinical systems such as EMIS or be willing to undertake the necessary training, this is essential to the role.
