Summary

Reablement Support Worker - Portsmouth

Location: Portsmouth

Hours: Full-time / Part-time

We are currently recruiting Reablement Support Workers to join our dedicated team in Portsmouth, supporting people in their own homes to regain independence following illness, injury, or a hospital stay.

This is a rewarding, hands-on role focused on helping people do things for themselves, rather than doing tasks for them. You will coach, encourage, and guide individuals to rebuild skills, confidence, and independence so they can live safely at home.

Key Requirements
  • Must hold a full UK driving licence and have access to a vehicle

  • Must have a valid Practical Moving & Handling Certificate

  • Must have DBS on update service
  • Right to work in the UK (we do not offer sponsorship)

  • Experience in care, reablement, or support work preferred

  • A positive, encouraging approach with good communication skills

Key Responsibilities
  • Promote Independence: Encourage and guide individuals to complete daily living tasks such as cooking, cleaning, personal care, and managing medication

  • Mobility Support: Assist with safe movement including getting in and out of bed, walking, and using stairs

  • Skill Rebuilding: Support people to relearn skills or adapt following illness, stroke, injury, or reduced mobility

  • Confidence Building: Empower individuals to feel capable, motivated, and confident in their own abilities

  • Follow Support Plans: Work to person-centred reablement plans and accurately document progress

  • Facilitate Discharge: Support safe and effective transitions from hospital or respite care back into the community

About the Role

You will work in people's homes, following structured reablement plans designed to reduce reliance on care services and promote long-term independence. Tasks may include support with personal care, mobility, household activities, medication prompts, and community access - always with a focus on encouragement, guidance, and skill-building.
