Summary
The assistant practitioner will work alongside colleagues to prevent hospital admission, improve ADL function and support post hospital discharge. You will be confident speaking to members of the public and be able to modify your communication style as required to the situation. You will have an understanding of falls prevention and rehabilitation, ideally with experience of community working and be able to provide care and support as required. You will act professionally at all times and demonstrate a positive image of the service. You will maintain accurate records and know your limitations and when to escalate risk and safeguarding concerns to the registered team members. You will support the team to patients and carers on the waiting list for the service. You will be involved in all aspects of the service and team working as required. During your induction, you will be supported by registered staff and full training will be given including monthly clinical supervision. Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies. A full UK driving license and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
