Summary
Job Title: Primary Care Clinical Coder / Summariser Location: Belgrave Medical Centre Reports to: Practice Manager / Clinical Lead Role Overview: We are seeking a meticulous and motivated Primary Care Clinical Coder / Summariser to join our supportive and dynamic team. This role is pivotal in ensuring accurate clinical documentation and coding of patient records, which supports patient care, audit, reporting, and funding. The ideal candidate thrives in a primary care environment, has strong attention to detail, and is committed to maintaining high standards of data quality. Key Responsibilities: Accurately summarise patient medical records, ensuring all relevant diagnoses, procedures, medications, and allergies are up to date. Apply appropriate clinical codes (Read Codes / SNOMED CT / GP codes) to patient records in line with NHS and local guidelines. Ensure compliance with data protection and patient confidentiality standards. Support GPs and practice staff with clinical documentation queries and coding guidance. Assist in audit and quality improvement initiatives through accurate data entry and reporting. Keep abreast of changes in clinical coding standards and primary care guidance. Work collaboratively with the wider practice team to ensure high-quality patient records and data integrity.
