Crynodeb
Working in the community, the post holder will oversee the management of inpatients on both rehabilitation and subacute pathways and provide medical input to the ARC a multidisciplinary clinic completing CGAs with patients referred by GPs, South Central Ambulance Service and secondary care. Working across the two trusts they will build relationships with acute medicine, radiology, frailty and community services, which will enable them to develop new pathways to support a seamless transition of care. This post is uniquely placed to advocate for the right care, right place, right time model of healthcare that underpins the care of older people. It aligns well with the NHS 10 year plan and offers the successful candidate excellent opportunities for career development as well as the chance to make a meaningful contribution to the care of older people across the wider system.
