Summary
Job Responsibilities: 1. To act as the Infection Prevention Control (IPC) lead and to work to the IPC policy, providing audits, training, support and evidence of compliance to management. Following that, produce the IPC annual statement. 2. The Lead Nurse will be CQC lead for the nursing team ensuring services provided are effective, safe, responsive, caring and well-led by using up to date policies and protocols. 3. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence based practice. 4. To develop, implement and embed health promotion and wellbeing programmes. 5.Management of chronic diseases which includes asthma, hypertension, COPD, CVA-TIA, CHD, and CHD risk assessment and management. Initiates the process of diagnosis with patients suspected to have chronic disease 6. To co-ordinate the provision of travel vaccination services. 7.To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required. 8. To maintain accurate clinical records in conjunction with extant legislation. 9. To ensure snomed codes are used effectively by all members of the nursing team. 10. To ensure compliance with the practice chaperone policy. 11. To support the team in dealing with clinical emergencies. 12. To recognize, assess and refer patients presenting with mental health needs. 13. To lead and implement vaccination program for adults and children, working with the wider team to increase uptake. 14. Ensure safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained. 15. Participate in the review of significant and near-miss events, applying a structured approach.
