Days and Hours of Work:
Working 3 consecutive days, 2 days 09:00-16:30, 1 day 09:00-12:30.
Days of working are negotiable, flexi time is applicable, and starting and finishing times are flexible.

The Hospital Discharge and Care Home Review Team is responsible for assessing and supporting complex discharges from hospital to home or care homes. The team also maintains oversight of all existing care home placements, including reviews, and responds to incidents including Adult Support & Protection. The team has a key role in reducing delayed discharge times and quality assurance within care homes.
Based within East Lothian Community Hospital
Hospital discharge and care home work
Working in hospital sites across the Lothians and care homes mainly in East Lothian, but potentially across Scotland
Undertaking work to enable patients to return home or move to care homes such as managing equipment, organising frozen meals or telecare and facilitating house cleans
Carrying out reviews and other work within care homes
Duty work responding to care home incidents
Interviews for this post will take place on 12/1/26 at East Lothian Community Hospital.

Co-ordinate the provision of a range of services to meet the assessed needs of individuals requiring assistant, to enable them to maintain and maximise their independence.

Job Requirements
Relevant Qualifications
Educated to HNC or SVQ level 3 in social care, or equivalent, and be able to demonstrate equivalent knowledge, skills and competencies gained through relevant experience.
Driving Licence
As the duties of this role require you to effectively travel to successfully undertake the full remit of the role, you must hold a current driving licence and have access to a vehicle.*
