Summary

Job Summary: Working under the direct supervision of the Practice Nurse and Assistant Practitioner and strictly in accordance with specific guidelines and protocols, the Healthcare Assistant will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care and lead on areas of the Salford Standard. Duties and Responsibilities New patient health checks ECG recording BP monitoring Urinalysis Phlebotomy Ensure specimens are labelled and bagged ready for collection with the necessary paperwork completed. Ensure clinical waste collection runs smoothly, reporting any problems to the Practice Nurse. Ensure sharps bins are ready for collection. Restocking/maintenance of equipment. Restocking of clinical areas and consulting rooms. Chaperoning duties Assisting the assessment and surveillance of patients health and well-being Helping to raise awareness of health and well-being and how it can be promoted Assisting with the collection and collation of data on needs related to health and well-being Provide lifestyle advice to patients making any necessary referrals within the practice. Home visiting housebound patients for reviews and blood tests. Specific Salford Standard KPIs: Health Promotion Raising awareness of prevention. Doing practice displays to highlight national health promotion campaigns. Devise a national calendar of health promotion events. Linking with the health improvement worker to ensure patients are using community assets for health improvement. Respiratory Risk questionnaire Identify patients aged over 40 who smoke and contact those patients to fill in the risk questionnaire, perform spirometry if a positive result and refer to the practice nurse. Encourage and chase up all children with Asthma to attend for their reviews. DNAs and Non-Compliance Chase up all patients who have not attended for all screening appointments, LTC, mammograms, cervical cytology, bowel screening and contact to see if there are any barriers to that patient attending. IGR Reviews Follow these patients up annually and ensure that lifestyle intervention is offered. Carers Increase the identification of carers within the practice using text messaging, campaigning in waiting areas and generally raising the profile of carers within the practices. Ensure every carer is invited for an annual carers review. Link with the carers centre and refer patients as required. Health Improvement Invite and perform NHS Health Checks and encourage those who do not attend to engage with this. Pulse checks for all patients over 65. Audit-C management for all patients over 16. Offer BP check for all patients aged over 30. Refer all patients who need lifestyle intervention to the health improvement worker in the community who will assess and refer on to other services which are relevant to the patients needs in the community. Lifestyle holistic reviews as part of the LTC. Health Protection Contact all at-risk and patients over 65 who do not attend for an influenza/pneumococcal vaccination.
