Summary
The BBV Health Adviser and Pathway Coordinator post is to provide clinical support and pathways into care for BBV (HIV, Hepatitis B, and C) positive patients tested under the Emergency Department Opt-Out BBV Testing Project.
• · Provide information for clients diagnosed with BBV infections, including HIV, providing support and onward referral as required
• · To facilitate the process of partner notification/management for these clients in order to encourage, with the client’s consent, the attendance of all potentially infected contacts to a GUM service.
• · To manage the patient pathway for all patients with reactive blood borne results who were tested in the ED departments.
· Data collection and regular reporting of the links into clinics for patients with a positive BBV test.
· To be responsible for tracking monitoring/treatment pathway for BBV patients.
· Collate reports and updates for the clinical leads when requested.

· Provide information for clients diagnosed with BBV infections, including HIV, providing support and onward referral as required
· To facilitate the process of partner notification/management for these clients in order to encourage, with the client’s consent, the attendance of all potentially infected contacts to a GUM service.
· To manage the patient pathway for all patients with reactive blood borne results who were tested in the ED departments.
· To develop and maintain supportive and effective relationships with the Hepatology team in the hospital.
· To exchange relevant information about clients and contacts with other Sexual Health clinics as necessary.
· To take histories from clients as appropriate and refer on to relevant individuals in the multi professional team as part of the care pathway.
· Provide initial HIV post-test discussion/support for those clients who have a reactive or positive HIV test result, ensuring they are linked in to HIV outpatient services and facilitating onward referral where necessary to other appropriate services.
· Where appropriate, supporting partners, significant others, and immediate family of those newly diagnosed with HIV, including facilitating partner notification, testing and disclosure.
· Maintain accurate client records, ensuring all entries are dated, signed and designation recorded as in accordance with Trusts policies and procedures
