Summary

Ensure that the team case load is kept up to date. To participate and co-operate with the Multi Disciplinary Team to contribute to the effectiveness of the service as a whole. To manage and maintain the teams clinical caseload, including all associated administration tasks. To use multiple patient information management systems (EMIS, Docobo, Patient Centre etc.) to enter such information as required on the computerised patient information system. To use discretion, sensitivity and deal with information in a confidential and professional manner at all times. Oversee and monitor the distribution/collection of equipment that is used to monitor patients in their own home (tech-enabled care). Maintain an accurate database of the home monitoring devices. To be responsible for data collection, based on the outcomes from the clinical caseload. To participate in and organise team audits when appropriate. If successful this vacancy will require an standard DBS check at a cost of £26.90. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
