A full job description is available on request Key responsibilities Long Term Condition Management - face to face or telephone consultations with patients on single or multiple medical problems where medicine optimisation is required. Review the on going need for each medicine and review of monitoring needs and an opportunity to support patients with their medication taking, ensuring they get the best use of their medicines. Prescribe medication for long term condition management following local and national guidelines, making appropriate recommendations to other clinicians for medicine improvement. Undertake clinical medication reviews with patients and alter medications to improve prescribing. Facilitate Group Consultations about long term condition medications and develop suitable resources for these. Provide patient facing and telephone consultation clinics for those with questions, queries and concerns about their medications Provide assistance and answers regarding relevant medicine related queries from GP's, other practice staff and other healthcare teams Review the use of medications associated with unplanned hospital admissions and re-admissions through audit and individual patient reviews Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge Produce and implement a practice repeat prescribing policy ensuring patients have appropriate monitoring tests in place when required Undertake clinical audits of prescribing in areas directed by the clinicians, patient complaints, new guidelines and significant event analysis Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components. This includes setting pathways for and implementing Service Level Agreements and Locally Enhanced Services Implement changes to medicines that result from MHRA alerts, product withdrawl and other local and national guidance.
