Summary
Applicants must have previous experience working in a General Practice setting and must be fully proficient in using the EMIS Web clinical system. These requirements are essential. We are seeking an experienced and highly skilled Practice Nurse to join our team. The successful candidate will provide high-quality clinical care, leadership, and support across our practices, with a particular focus on chronic disease management and service development. Key Responsibilities Deliver comprehensive nursing care and evidence-based clinical advice to patients, including the management of chronic conditions such as asthma, diabetes, and hypertension. Travel to our Tameside practices at least twice per week to provide on-site leadership, operational support, and mentorship to the nursing team. Oversee nursing performance, ensuring consistently high standards of clinical care, continuous professional development, and compliance with practice policies and national guidelines. Attend weekly managerial meetings in Tameside (held once per week in the morning). Conduct health screenings, childhood and adult immunisations, and other routine vaccinations. Perform cervical cytology and actively participate in the cervical screening programme. Provide health promotion, lifestyle guidance, and behavioural support, including smoking cessation and weight management advice. Support and contribute to the achievement of Quality and Outcomes Framework (QOF) targets. Work collaboratively with GPs, healthcare assistants, and the wider multidisciplinary team to deliver coordinated and patient-centred care. Maintain accurate, timely, and compliant clinical documentation in accordance with practice policies and regulatory requirements. Lead and oversee Infection Prevention and Control (IPC) across the surgery, ensuring full adherence to statutory standards and best-practice protocols.
