Summary
We are working closely alongside with a local authority in Cardiff to assist with the appointment of a Reablement Care Worker on a 3-months contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Summary:
The Reablement Care Worker is a vital frontline role within the Cardiff Council's Community Resource Team (CRT) Homecare service, working in partnership with Cardiff and Vale University Health Board. The purpose of this role is to provide short-term, intensive, and outcome-focused support to service users in their own homes, typically following an illness, injury, or hospital stay.The core objective is to enable citizens to learn or re-learn the necessary skills for daily living, maximizing their functional independence and supporting them to achieve their wellbeing goals. This service is crucial for preventing unnecessary hospital admissions, facilitating timely discharges, and delaying or reducing the need for longer-term social care services.
Responsibilities:
Undertake delegated health care tasks as required, including prompting or assisting with prescribed medication, applying prescribed creams/drops, and prompting exercises as directed by a Physiotherapist.
Identify, record, and report all incidents, accidents, potential safeguarding, and other high-risk situations promptly. Respond to emergency situations by taking appropriate action.
Maintain good observation and recording skills. Provide written observations on the service users circumstances, including improvement or deterioration, to assist with the review of the reablement program.
Maintain accurate daily records using the provided mobile device that allocates daily tasks.
Qualification:
Willingness and commitment to train to achieve the relevant HSc (Health and Social Care) level required for the post (previously NVQ/QCF).
Essentials:
A caring nature and a strong commitment to promoting independence and wellbeing.
Experience in providing personal care and support, either in a paid or voluntary capacity.
Commitment to register with Social Care Wales and adhere to its Code of Professional Practice.
Excellent communication skills (verbal and written) and the ability to converse at ease with customers.
