Summary
The role of the pharmacist based at Iridium Medical Practice B33 8TA will include: Structured Medication Reviews (SMR) Long-term conditions (LTCs) management Patient-facing role Management of acute and repeat requests Dealing with medication related queries Care Home Medication Reviews Complex Polypharmacy management Elderly and frail patients care Quality Improvement Prescribing Indicators (QOF) Specialist expertise of medicines Lead on person-centred Medicines Optimisation Lead on local Anti-Microbial Stewardship (AMR) within the PCN QOF contribution and Enhanced Services Work with Pharmacy teams across Primary Care, Community and Hospital as part of the wider healthcare team to improve patient outcomes
