Summary
Assist with education and support of patients with care of tracheostomy, weaning and decannulation. Responsible for accurate, timely documentation of clinical plan in patients records both computerised and paper based. Utilise and ensure others utilise information and IT systems to secure accurate and timely patient, workforce, and resource data available. Identify and implement, in collaboration with the multi-professional team, appropriate areas for research and audit. Establish and maintain professional networking relationships between departments in the trust to promote best practice in the trust. Support staff, patients and family during difficult situations arising in the clinical area, e.g. breaking bad news or following an unexpected event.
