The Children's Kidney Unit has a new opportunity for an experienced Paediatric Renal Nurse to join the Chronic Kidney Disease team.
The CKD Nurse Specialist will work alongside a multi-professional team caring for children with CKD stages 3 - 5. The nurse will be responsible for the preparation, assessment, training and follow-up of children and their families.
The ideal candidate will have experience of working in paediatric nephrology.
Management of caseload of CKD patients
Preparing children and their families for transplant listing.
Organising regular follow-up via telephone and face to face clinics, home visits, and shared care clinics.
Undertaking home assessments.
Training patients and carers to undertake related therapies.
Liaising with primary care providers, schools and local authorities.
Coordinating supplies and medication.
Be responsive to family advice calls, trouble-shooting, and escalating concerns.
Work regular shifts in the children's haemodialysis unit.
