Summary
A Care Plan Coordinator is responsible for managing patient care, coordinating treatment plans, and ensuring effective communication between Residents and healthcare providers.
Key Responsibilities
Patient Coordination: Work closely with patients to assess their needs, develop personalized care plans, and coordinate services to ensure they receive appropriate care.
Collaboration: Collaborate with healthcare teams, including doctors, nurses, and social workers, to implement and monitor care plans.
Monitoring Progress: Regularly check in with patients to evaluate their progress and adjust care plans as needed.
