Summary


The Navigator will work autonomously, possess excellent communication skills and have experience and knowledge of working with Crisis/Rapid Response and community services within primary or secondary care services.

The post holder will be expected to undertake the holistic assessment of patients using a person-centered approach, and no-ordinate care packages across both health and social care as appropriate to avoid inappropriate admission to hospital or long-term care environments, in addition to facilitate the timely discharge of patients from secondary care. The post-holder will provide holistic assessments, diagnose and formulate individual treatment plans and programmes to meet the needs of patients who are referred to the navigator service.

The post requires experience and knowledge from a health related professional or Allied Health Professional, The Navigator will be based in the A&E department and support emergency and primary care staff by identifying, sign-posting, assessing and coordinating patients deemed appropriate to other alternative community and outpatient services. Close liaison is required with clinicians and professionals working in emergency care within the hospital A&E department and Medical Assessment Unit

To work as a practitioner, incorporating your specialist skills alongside nursing and clinical staff within the emergency departments. Actively seeking to present patients that may require sign-posting to Crisis Response, Urgent Community Care, community services, social care and/or specialist services.

To provide holistic assessments, including mobility and functional assessments. To diagnose and formulate individual treatment plans and programmes to meet the needs of patients who are referred to the service.

To use knowledge, advanced clinical reasoning and problem-solving skills to analyse clinical and non-clinical findings to form accurate diagnoses and prognoses in a wide range of complex conditions. This may include the use of standard tests to support diagnosis.

To identify and manage risk with regards to function, mobility and cognition.

To act upon findings; recommending and implementing interventions, providing/supporting comprehensive case management, treatment and discharge plans accordingly. This will require analysis and interpretation from a wide range of options.
