Summary

Work as an autonomous practitioner within the integrated team providing rapid and planned assessment and treatment interventions to patients in their own homes or other community settings to enable patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible. This will be achieved through a combination of hands on clinical skills and knowledge to treat, educate and support patients on a designated caseload and by working with patients and their carers to assess, plan and implement appropriate packages of care. KEY RESPONSIBILITIES The post holder will have competencies in all of the following areas and will: Clinical Work within the NMC Code. Be responsible for the initial assessment and review of patients, across a broad spectrum of clinical conditions, considering the holistic needs of patients, under the direction of the senior nurse. Provide clinical care, treatment, advice and interventions, including the transcribing and prescribing of medication if appropriate, through holistic assessment of patient need; and then planning, implementing and evaluating a person centred care plan as appropriate. Be responsible for admitting and discharging patients from the caseload. Undertake continuing health care checklist and fast track applications of on-going nursing need and where eligible commission packages of care from external agencies. Ensure clinical practice is patient-focussed and evidence-based, in accordance with NMC code and scope for professional practice. Assess mental capacity and have the ability to work within a legal framework with patients who lack capacity to consent to treatment. Actively contribute to the task of monitoring and evaluating standards of patient care, seeking to continuously improve the quality of care. Ensure care provided maintains the individuals privacy and dignity, values diversity and respects cultural and individual rights and choices.
