Summary
Support medication reviews and medicines optimisation under the supervision of clinical pharmacists and GPs Process repeat prescriptions, ensuring accuracy and safety Reconcile medications following hospital discharges and outpatient appointments Provide advice to patients on the safe and effective use of their medicines Assist with audits, quality improvement projects, and prescribing initiatives Maintain accurate records and contribute to the smooth running of the practices medicines management systems
