We are seeking a dedicated Specialist Nurse to join our supportive Mental Health and Learning Disabilities (MHLD) team in East Kent.

To work as a specialist nurse within the MHLD team. The post holder will work with people with a learning disability whose care requires co-ordination due to complex needs.

The post holder will support the development of mainstream clinicians skills in assessing and treating mental illness in people with Learning Disability.

- The post holder will be responsible for maintaining joint working practices between MHLD, in-patient, mainstream community mental health services and CLTD.
- The post holder will work preventatively and supportively with people with a learning disability and mental health problems so that they enjoy a quality of life and maintain their place in the community.
- The role will require working closely with our main stakeholders to provide an enhanced package of care that supports the management of mental health issues in a way that avoids the need for inpatient admission and prevents placement breakdown.
- The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010)

· Operate as a Specialist Nurse for CPA level clients (and where appropriate non-CPA level clients) with complex and diverse needs.

· Undertake comprehensive assessments, developing and reviewing care-plans.
