Exciting things are happening in Adult Social Care on the Isle of Wight. Our service reflects our community-focused, strengths-based approach and brings key teams closer together.

As a result, we are looking for two Social Workers to join our Wellbeing & Access Hub, supporting people at their first point of contact. This is a dynamic role working alongside Outreach and Occupational Therapy colleagues to reduce the need for long-term care. You’ll help people regain independence through reablement, provide clear information and advice, and signpost to a wide range of third-sector services.

As part of the front door to Adult Social Care, you’ll handle telephone referrals, carry out visits, and complete Care Act assessments. You’ll deliver timely, strengths-based assessments and support plans that promote independence, choice and autonomy.

You’ll also play a key role in safeguarding, follow departmental recording standards, and contribute to multi-agency discharge planning to support safe, timely transitions. Supporting carers is central to the role too, including offering Carers Assessments in line with the Care Act 2014

Joining us means becoming part of an organisation that champions strong leadership, reflective supervision and a healthy work–life balance. We’re looking for social workers who share our ambition, passion and commitment to helping people live their best lives. You’ll bring confident, thoughtful practice and act as a strong advocate for high-quality Adult Social Care on the Isle of Wight.

In return, we offer a supportive environment with regular supervision, ongoing professional development, and meaningful training opportunities to help you grow and thrive.
