This role is part of a one-off project designed to improve hospital discharge outcomes and reduce delays, ensuring residents are safely and appropriately housed following hospital treatment.

The Role
As a HOST Casework Officer (Housing Hospital Discharge Coordinator), you will provide housing options, advice, and assistance to patients in hospital settings. You will ensure safe and timely discharge into suitable accommodation for Tower Hamlets residents, working closely with hospital teams and housing services to reduce demand on hospitals and social care.

Key Responsibilities
Work closely with hospital teams to facilitate discharge planning.

Apply detailed knowledge of homelessness legislation, including the Homelessness Reduction Act 2018.

Coordinate referrals to boroughs of residence and liaise with hospitals such as Royal London, Newham, and Mile End.

Provide tailored housing advice and options to patients.

Collaborate with multi-disciplinary teams across hospitals and housing services.

Ensure compliance with statutory duties and council policies, including Customer Care, Equal Opportunities, Data Protection, and Health & Safety.

Maintain accurate case records and use case management systems effectively.

Work proactively to prevent homelessness and secure sustainable housing solutions.

Candidate Requirements
Eligible to work in the UK.

DBS check required.

Declaration of Interest – Tower Hamlets.

Strong experience in housing and homelessness services, including hospital discharge planning.

In-depth knowledge of homelessness legislation (Homelessness Reduction Act 2018, Housing Act 1996 Part 7, Homelessness Act 2002).

Experience working with homeless people and understanding complex needs (including substance misuse).

Advanced partnership working skills with health, social care, and housing stakeholders.

Excellent communication and advocacy skills, able to support vulnerable individuals.

Strong problem-solving abilities under time pressure.

Experience using case management systems and maintaining accurate records.

Working Environment
You will work within Tower Hamlets’ housing and regeneration framework, combining hospital-based collaboration with council housing services to deliver effective outcomes.
