Summary

Using expert knowledge, advanced skill/ practice and autonomous decision-making skills, manage a caseload of patients in the defined client group. Comprehensively assess, review and evaluate the needs of both patients and their carers to improve their physical and psychological well-being whilst managing exacerbation of underlying conditions and reducing the need for avoidable acute hospitalisation in relation to their physical health condition. Work in partnership with patients, carers, consultants, medics, and other health professionals as appropriate to instigate diagnostic testing and therapeutic treatments, ascertain diagnosis, develop, initiate and implement proactive care plans. Use multi-disciplinary ward rounds to optimize patient outcomes and treatment plans. Use systematic processes of reasoning and reach justifiable conclusions, including the making of differential, definitive diagnoses within knowledge base and patients long -term conditions. Monitoring individuals and reviewing individualized care plans. Monitoring individuals and reviewing individualized care plans. Use advanced clinical skills to undertake interventions consistent with evidence-based practice in line with national and local policy. To practice as an independent prescriber, using knowledge of medicines and prescribing skills to influence the reduction of risk and complications and associated with medication and polypharmacy. To work closely with medics and prescribing advisors and pharmacists as appropriate. The post holder will incorporate the concept of self-care wherever practicable (depending on client group and individuals level of cognition) into their practice and will be responsible for ensuring that all staff within their sphere of responsibility promote good health and enable patients towards self-care, independence and recovery. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
