Key Responsibilities

Conduct structured medication reviews for patients with complex needs and polypharmacy
Lead proactive medicines optimisation and medication safety initiatives
Serve as a trusted and informed point of contact for medication-related queries
Support delivery of QOF, IIF, CQC compliance, and local commissioning targets
Engage in repeat prescribing processes and clinical audits
Deliver face-to-face and virtual patient consultations
Build strong relationships across wider PCN partners and community services
Support chronic disease management pathways and personalised care plans

Ideal Candidate

GPhC registered with strong clinical knowledge and professional integrity
Demonstrates excellent communication and patient engagement skills
Confident in autonomous clinical decision-making within agreed protocols
Experience within primary care is highly valued, though not essential
Enthusiasm for continuous learning and professional progression
