Summary


This role focuses on tracking and managing the referral and treatment process for cancer patients, ensuring that those referred under the Suspected Cancer Pathway are diagnosed promptly and that newly diagnosed patients begin treatment within national timescales- Cancer Guidelines V12.1. The individual will be responsible for maintaining accurate tracking systems and producing weekly reports to monitor patient progress. They will also liaise with MDT Navigators across SWL network to ensure that patients requiring treatment are flagged and treated accordingly.



In addition to referral and treatment tracking, the MDTC are expected to be involved in capturing and updating patient information in Trust databases, ensuring all relevant details are recorded and up-to-date. The role requires collaboration with the Senior MDT co-ordinators to develop and monitor national and local datasets, including tracking waiting times for newly diagnosed cancer patients. Timely reporting on these metrics is essential to ensure all patients are receiving care in a timely manner

This post is for Monday-Friday between 08:00am and 17:00pm on a rota basis. Flexible working cannot be considered for this role.

Operational Responsibilities/Support to Departments

1. Assist with the regular review of Multidisciplinary Team Operational Policies and Procedures.

2. Supporting the Cancer Services Management team in the delivery of Cancer National Waiting times and Breaches workshops.

3. To support Cancer Services Management Team in delivering Cancer audits, COSD, staging target CQUIN for the year.

4. Responsible for investigating Breaches of the Cancer Waiting times and for providing the feedback to involved parties.

5. Responsible for producing the resulting Breach Reports, which are submitted to the CSU (Commissioning Support Unit)/CCG(Clinical Commissioning Group) in accordance with reporting procedures.

Data Co-ordination and Tracking Cancer Patients

1. To track all Two-Week Rule referrals to ensure that suspected cancer patients receive prompt diagnosis, and to work with other Trust staff to bring forward appointments if necessary.

2. To update tracking systems daily to indicate whether Two Week Rule patients have a benign or malignant diagnosis.

3. To work with Clinicians, Clinical Nurse Specialists, schedulers, diagnostic staff and other cancer data co-ordinators to ensure identification and recording of the Trust’s newly diagnosed cancer patients.

4. To track all newly diagnosed cancer patients to ensure that they are treated within national timescales, and to work with staff from this Trust or others to book or bring forward appointments as necessary.
